Gastro esophageal reflux disease








 An uncomfortable burning sensation along the middle of the chest that may extend to the neck or neck immediately after eating, in the afternoon or at night while lying down or working while leaning forward. 

It may cause bitter or sour taste in the mouth. 

This condition can last from a few minutes to several hours.




Experiences like this happen to men and women, young and old, at some point in their lives, and it is unlikely to be anything to worry about.



At present 20% people in Bangladesh are suffering from this disease.



Heartburn is a major symptom of GERD.

It is a preventable disease.




The cause of this disease:


=>Obesity and weight gain,

=>During pregnancy,

 =>hiatus hernia,

=>GERD

=>Smoking, drinking;

=>Insomnia, waking up late at night,

=> Lying down immediately after eating,

=>Carbonated soft drinks, tea, coffee,   chocolate;

=>Some drugs - aspirin, painkillers,

 =>Excess fast food and junk oil and fatty         foods.



Symptoms of the disease


=>Heartburn,

=>Food rises to the top,

=>Chest pain or abdominal pain,

=>Nausea,

=>Excess saliva in the face,

=>Problems with swallowing food,

=>Bad breath,

=>Dry cough.




The regulators that cause heartburn



=>After eating onions,

=>After eating sour fruits,

=>High fat foods,

=>Tomatoes,

=>Foods made with tomatoes,

=>Alcohol,

=>Sour fruit juice etc.




What can happen if proper treatment is not given




★ Inflammation of the esophagus or Oesophagitis,


★ Erosion of the esophagus,


★ Narrowing of the esophagus,


★ Long-term sore throat


 •Change of voice (hoarse voice),


•Shortness of breath,


•Tooth decay and gum problems,


•Sudden sleep deprivation due to food rising to sleep,

★ Esophageal cancer





What to do to cure the disease


=>Maintain the desired body weight,

=>Avoid overeating or overeating,

=> Make it a habit to eat little by little for a     while,

=>Refrain from eating whole foods together,


=>Eating moderately pure and balanced food,

=>Keep the head of the bed elevated or use two pillows instead of one.


=>Make it a habit to finish dinner at least 3 hours before going to bed,


=>Avoid lying down immediately after eating,


=>Wear loose clothing,


=>Avoid extra fast food, gurupak and bad oil foods,


=>Quit smoking and drinking,


 =>Exercise regularly every day;


 =>To live a happy and contented life with a smile from being engaged in religious and creative work and charity.






Management / treatment



1. Lifestyle advice, including


            => Weight loss,

            => Avoid dietary items

              that patient's condition worsens

 

             =>The height of the edge of the head

                   bed in those experiences

                    nocturnal symptoms,


             =>Avoid late meals and

              =>Quit smoking 



2. Antacids and alginates (1 0-20 ml 3 times after meals and at bedtime)



3. When dysmotility is characteristic

Prominent, domperidone (up to 10 mg)

Three times daily) may be helpful.



4. Anti-H2 receptor drugs also help

Symptoms but do not cure oesophagitis.



5. The proton pump inhibitor (PPI) is

For severe symptoms and complex reflux

Treatment of choice.



6. Laparoscopic anti-reflux surgery (90%

Success): Indications are not clear

But for patients who may-


Failure to respond to medical therapy,

Reluctant to take long term
